I allso had this problem, but it was something with my DVD player firmware. becouse after I installed 1.01 (wich improves DVD sync) it worked, so I knew my DVD had some syncing errors and after a long time of searching finally found DVD firmware.
becouse it's rare... depends on the map. if someone makes a map with veary bad placed spawns we all will get this bug if we play on it
hmm nice idea for a new map "now you see me, and now not anymore..." or what about. "I hear you but I dont see you..."
everyone can get this bug it just depends on the map you're playing on

that your DVD is messed up is not caused by a patch...
patching simply can NOT affect your disk.. you must have done something else stupid with your DVD. so why exactly can't you reinstall?
just uninstall GTA:SA using 'add or remove programs' (in your controll panel) then trash the GTA folder in your program files ('bad' files my still be in there) and then run the setup again from the DVD...
you can improve your internet connection if you run on xp pro.
windows takes some bandwith for itself for unnessesary things... to disable this:
start-->run-->gpedit.msc
computer configuration-->administrative templates-->QoS packet sceduler
and set Limit reservable bandwith to 0
would be cool...
