Activate this in the Admin Control Panel in the Captcha Settings section: http://recaptcha.net/

I have used both for a very long time on multiple forums, PHPBB included, and they work well. The Recaptcha nails most of the spam bots, and the Stop Forum Spam mod catches the very few that leak through and stop them before they can activate themselves via email.
