If you're talking about having NPC's (non-playing characters such as pedestrians), then this isn't possible. The current release of MTASA allows only racing (you against other people on the Internet), but the development team are working on creating a Deathmatch mod too (so you can run around and kill other people who play over the Internet). However, the date when this will be released is still unknown, although it's highly likely it will not be in the near future.

ok u wish it would come out soon tho cos the reason i joined this was t play gtasa online shootin other ppl all over 4 islands it would be well fun but this is a big let down cos in the pics it show foot and viechel fighting but u cant wats the point

It all depends on the development, if there are certain setbacks then it haults the progress slightly, which then stops the modification being released any time sooner then what was originally expected.
